There is a clear consensus that redesigning services around the needs of individuals in a ‘place’ provides the best opportunity to improve people’s health and wellbeing, address health inequalities, and help achieve financial sustainability.

But what progress has been made to date and what more needs to be done?

We’ve teamed up with #Confed2016 media partners the Health Service Journal and Local Government Chronicle to gauge the views of local leaders across health and care on the current state of service integration.

We’re seeking local leaders’ views on integration where they are, their assessment of current demand for services and the factors that could improve integration in the future.

Delivering joined-up care

How areas can deliver more joined-up care at local level is a key theme at our annual conference and exhibition– the premier gathering of health and care leaders, decision-makers and partners in the UK.
