Harrogate and Rural District CCG: Linking GPs with care homes

Staff at Harrogate and Rural District Clinical Commissioning Group (CCG) identified that local care home residents had twice the number of GP visits as other patients of a similar age. Many of the requests for visits were chaotic and unplanned, and could have been avoided and the problem more appropriately managed.

GPs had a small number of care home patients across a large number of facilities. This was an inefficient way of working which impacted on the quality of relationships between doctors and care home patients.
