Lincolnshire Community Health Services NHS Trust: Reducing delayed transfers of care in community hospitals

In April 2016, delayed transfers of care (DTOC) within the trust’s community hospitals stood at 16.2 per cent – a figure significantly higher than the national standard of 3.5 per cent. The problem was compounded by the demography of people using health services, predominantly the frail elderly with complex needs, and systemic issues relating to patient flow.

The trust resolved to take action to improve both the quality and experience of patient care and support system-wide improvements in patient flow.
