Health organisations in Lincolnshire were working in silos in responding to urgent care needs, leading to duplication in activity, disjointed patient journeys and patients unsure of appropriate service entry points.

There was a need for a multi-organisational approach to reduce unnecessary practitioner home visits, A&E attendances, emergency admissions, ambulance transportations, and to improve and integrate community care and a scarce and valued GP and practitioner workforce.
