Sharing the learning from the vanguard programme

Vanguard animation 800 x 400
We're working with NHS Clinical Commissioners, NHS Providers and the Local Government Association to help spread the learning from the vanguard programme across the health and care sector.

Together, we aim to create greater understanding, involvement and ownership of the vanguard vision, showcasing new ways health and care economies can help establish a sustainable health service now and in the future.

Site visits

We're undertaking a series of site visits to localities involved in the vanguard programme. The visits aim to help share the learning of developing new models of care across localities.

Each visit includes:

  • an overview of the model and health system and motivations for integration
  • discussions with the senior team involved in development
  • a tour of local services.

The first visits took place between April and August 2016:

  • April with Dudley Multispecialty Community Provider.
  • May with Connecting Care Wakefield (care homes) and West Wakefield Health and Wellbeing Multispecialty Community Provider.
  • May with Whitstable Medical Practice Multispecialty Community Provider.
  • August with Northumbria Foundation Group, an acute care collaboration vanguard.
