Digital technology has improved integrated relationships across primary and secondary care and provided patients with a more personalised ‘at home care’ through the concept of a virtual ward, thus enabling patient empowerment and education through self-monitoring.
The webinar also demonstrates the new Inhealthcare digital dashboard which supports and prioritises homebased patient care in the community.
