Addressing the wider determinants of health: Exploring the impact of social prescribing on health inequalities

In collaboration with NHS England, this latest session in our health inequalities series explores how the increased skill mix provided by the additional roles reimbursement scheme (ARRS) enables general practice to support patients with the wider determinants of health.
Health inequalities are often exacerbated by the wider determinants of health which have a significant impact of patient outcomes but have traditionally been beyond the remit of the GP.
Join us and the team from The Ridge Medical Practice and Five Parks PCN to learn about their alternative appointment model for high intensity service users which utilises their social prescribing team in collaboration with GPs to address complex social issues faced by patients.
