Colleagues from Humber and North Yorkshire Health and Care Partnership join a panel chaired by Professor Aruna Garcea. They will discuss how primary care networks and secondary care within the Humber and North Yorkshire Integrated Care Board have collaborated to transform the cardiology pathway. They'll share how they have leveraged technology, estates and new relationships to push forward with this new way of working.
The webinar will demonstrate what an integrated primary and secondary care outpatient model could look like that supports the clinical behaviour essential to transform our system and better meet the needs of our population.
