Interface improvement hub | NHS Confederation

This hub contains tools, guidance, templates and tips to support improvement in organisations working across the interface between primary and secondary care. The content is based on the NHS Confederation's primary and secondary care interface collaborative, which ran from April to November 2024, in which teams of healthcare professionals came together to co-create and design innovative approaches to overcome challenges around interface working within their local areas.
