In conversation with Francesca Reville, senior programme manager at the NHS Confederation, Lesley and Zoe discuss some of the initiatives that are improving the way patients access care including the red bag scheme, which improves access to patient records and ensures patients only have to tell their story once throughout their care journey. They also highlight the portrait of a life scheme, developed in partnership with the local mental health provider.

Lesley and Zoe explain how the initiatives have impacted patients and the findings of a year-long evaluation. They also share their advice for colleagues planning similar schemes.
