Advanced paramedic rotational placement models in Wales

How experience in primary care is empowering and informing decision making for advanced paramedics and reducing avoidable conveyance to hospital.

Overview

  • We spent time with advanced paramedics undertaking rotational placements in North Wales, where emergency staff rotate between spending time working in primary care settings, and the rest of the time working on call for the Welsh Ambulance Services University NHS Trust, all within the same geographic area. 
  • The placements develop a deeper understanding of the ways in which patients can safely be managed within the community through alternative pathways, upskilling staff and providing them with a wider range of options when attending emergency calls. It also provides them with a more detailed understanding of the needs and history of particular patients who they may be encountering across both settings. 
  • Advanced paramedic practitioners who have benefitted from learning through the rotational placement model have lower rates of conveyance into hospital, at around 30%, compared to 60% for other paramedics. This means that patients are more effectively directed to appropriate services, while at the same time pressure on emergency departments is partially alleviated.
