Join us in conversation with NHS England to discuss the latest updates on implementing the Core20Plus5 framework. This session will explore the theory and practicalities behind the framework, and the major conditions strategy through the lens of cardiovascular disease care.
We will be joined by Professor Bola Olowabi and the Erewash PCN team, who will share examples of good practice and explore solutions to the challenges of designing and implementing effective health inequalities initiatives.
