They are one of the first Trusts in the country to operate primary care services.
The RWT vertically integrated primary care model went live in June 2016. Starting with three GP practices the model has grown and they now operate eight practices covering a population of 50,000 registered patients. Just made it third person to match up with the rest of the description
As an early innovator there was no road map for how to do this and have had to learn as they go! Forging new approaches, innovating to overcome barriers, and of course getting it wrong sometimes. With the benefit of their experience, we invite others to come and hear how to avoid some of the pitfalls, fast track some of the work and decide if the model looks like an approach that would work for patients and staff in your area.
